HCV Recertification E-Guide – Step 2

There are five document types required in advance of your annual appointment. Use the checklist below to prepare these documents before your annual interview. See your Notice of Annual Interview for the date documentation is due to your caseworker.

Doc Type: Income
For Whom? All Household Adults
  • Paystubs (For weekly pay – 6 paystubs, For bi-weekly pay – 3 paystubs)
  • Unemployment income
  • VA income
  • Child Support
  • TANF (cash assistance)
  • Food Stamps
  • VA income
  • any and all other income
Doc Type: Assets
For Whom? All Household Members
  • 3 months of Bank Statements
Doc Type: Tax Returns
For Whom? All Household Adults
  • Prior Year’s IRS Form 1040 or 1040EZ
Doc Type: Medical Expenses
For Whom? Elderly/Disabled Head of Household Disabled Spouse
  • Receipts for paid medical expenses from the past year
  • Pharmacy documentation
Doc Type: Childcare Expenses
For Whom? All Members of Household
